* Dr Rindfleisch has a Chronic Care Management service set up with medicare (and some commercial insurances) to give those patients the extra care they need. He has on staff a chronic care specialist whom these patients have direct access to Dr Rindfleisch through her. This gives patients the ability to address any concerns or questions relayed directly to Dr Rindfleisch without delay. This specialist will make phone calls to home health agencies, nurses, care facilities, and even the patients to help coordinate the care needed and report to Dr rindfleisch on a monthly basis. Sometimes patients have a difficult time making it to the doctor's office. This program helps to conveniently coordinate care and helps to reduce the burden of trying to get to an appointment so frequently, and also diesigned to keep his patients from hospitalizations by addressing concerns sooner and on a more regular basis.
